The summaries are our own plain-language paraphrase. For legal matters we use the source's own framing: an allegation is an allegation, a fine is attributed to the regulator that imposed it, and a settlement is not a finding of wrongdoing (several were resolved with no admission of liability). ProPublica: state regulators, including California's DMHC, have rarely penalized insurers for inaccurate "ghost network" directories — roughly $82,500 in directory-error fines over about eight years.
CalMatters: estimates that a third or more of California directory listings may be wrong; the DMHC issued eight penalties totaling $152,500 under the directory-accuracy law. A bill to require 95% accuracy was withdrawn late in the 2025 session.
Federal HHS OIG report (OEI-02-23-00540): many plans' behavioral-health networks include "ghost" providers no longer practicing or unwilling to see enrollees, making networks look larger than they are. OIG urged CMS to improve directory accuracy.
