How this works
A fair, documented review process
This channel is for representatives of health plans, dental plans, and provider organizations (for example, Health Net, Molina, Liberty Dental, DentaQuest, and others). Tell us what you believe is inaccurate or what you would like removed, point us to the specific page, and explain why. We will review your request against our sources and respond.
Reviewed on our own schedule
We read every request carefully against the public record — but on our own timeline. Unlike a health plan answering a grievance, NoSuchProvider.org is under no obligation to respond within 30 days, or at all. For perspective: Health Net has now left its own matter unresolved for 0 days and counting. Submitting this form does not, by itself, create any obligation or relationship, and does not guarantee a particular outcome.
What helps us act quickly

- What specifically is inaccurate, and the correct information with a source if possible.
- For removal requests, the basis for removal.
- Your company-provided email, so we can verify your affiliation and respond.
